New care navigation program for Bourke

The NSW Outback Division of General Practice (ODGP) has launched a new program in Bourke called the ‘Bourke Pathways Care Navigation Program’ – designed to support residents of Bourke Shire in accessing existing health, community, and social services.

The Bourke Pathways Care Navigation Program is funded by nib foundation and the aim of this pilot program is to improve health outcomes and to support self-management and health literacy.

The Outback Division’s Corporate and program Manager Tracey Coyne said the ODGP was excited to launch the service.

“Whether it’s assistance with referrals, paperwork, or simply knowing where to turn for help, the program is here to ensure that the community receives the care and support it needs,” Tracey said.

“This free program is now operational at ODGP’s main street office in Bourke and is available to eligible local residents who may be experiencing difficulty navigating the complex landscape of health and social services.”

With many community members unaware of available services or unsure how to access them, this new initiative will bridge the gap by guiding individuals through the process, offering personalised support and referrals. […]
